Cayuga County Clerk Office - Cayuga County/State of New York Affidavit Concerning Survey Map

Our Affidavit Concerning Survey Map form is to be used when you file a stand-alone survey map in our office. There is no charge for the Affidavit Concerning Survey Map form, however, there is a $10.00 fee for filing a map. If presented at time of filing, addititional identical original maps can be file stamped at no charge.

If you are mailing your maps, be sure to place your Affidavit Concerning Survey Map form on top, along with your fees.

MAP FILING REQUIREMENTS:

• 2 paper maps (originals required)
• 1 mylar, canvas or cloth map (original required)
• All maps must be identical.
• Completed and signed Affidavit Concerning Survey Map form
• Appropriate surveyor, town/village and health department stamps/signatures
• Appropriate fees
