The Long Term Care Access Office, which is part of the Department of Health and Human Services, is unique in the fact that it combines staff from both Health and Human Services as well as staff from the Office for Aging.

In one office, we coordinate a number of county-run programs dealing with long term care, including administration of most Medicaid homecare programs and the Office for Aging homecare program (EISEP). The Office also offers Cayuga County residents assistance with planning for and obtaining cost-effective long-term care services for themselves or someone they know, regardless of age or income.

Our Health nurses will conduct a free in-home assessment and develop and individualized care plan for any County resident who need long-term care services. We also provide free screening (PRIs) for nursing home placement for County residents who need that level of care and aren't hospitalized.

Individuals who contact our office are also offered information and referral services. The Office also provides a nurse liaison at Auburn Memorial Hospital to work closely with the social work office to facilitate discharges.

We will be adding a new Office for Aging specialist this summer to provide support, education and information services to caregivers and to administer our respite program. Anyone can call our office for information or assistance. A referral from a physician is not necessary.
